Project goals

ZLG approach

Training came first but was deliberately built from the client’s own defect data — the 5-Why and fishbone exercises used their actual complaints, not textbook examples. Delivery ran bilingual across both sites. Then came the part that makes training stick: five months of live 8D coaching, where ZLG facilitated weekly team sessions on the open CAPA backlog, pushing every analysis past “operator error” to the process condition that made the error possible, and setting effectiveness verification criteria at initiation rather than closure.

Implementation and obstacles

The first obstacle was backlog triage — 63 open CAPAs could not all get 8D teams. We consolidated them into 17 problem statements by defect family, which itself revealed that three fixture-related causes were generating most of the pain. The second obstacle was verification data: two defect families had no inspection data granular enough to prove improvement, so inspection checkpoints were redesigned before fixes were even attempted.

Lessons learned

CAPA systems fail at the two ends nobody audits closely: problem definition and effectiveness verification. Once verification criteria were defined on day one of each 8D, weak root causes exposed themselves — the fix either moved the metric or it didn’t.
